Enrollment Procedures

Prior to placing your child in our care, we would like you to meet with the director, tour our facilities and ask any questions that you may have. Contact us to set up a time.

To apply, you must complete and return the following forms:

Application
Parent Information Form

Upon acceptance and enrollment, the following forms must be completed and returned:

Medical Release Form
Child’s Health Form for Admission (DSS Form 2900)
Certificate of Immunization (provided by your pediatrician and updated regularly in accordance with the immunization schedule)

First-time mothers are encouraged to spend time in our Infant Room during their maternity leave. This will allow the new mother to appreciate the care her child will receive and become better acquainted with our procedures. Please let us know when you would like to come.
