Diploma / Certificate Replacement Request a replacement for a previously issued diploma or certificate, mailed directly to you. Request a Transcript Complete a transcript request form for the Registrar’s Office to have your transcript mailed directly to the place designated. Apostille / Authentication Complete an apostille / authentication request form for the Registrar’s Office to authenticate your documents for use in foreign countries.
